📝 Are signed Authorizations Required?

YES – A signed authorization from the applicant/driver is required prior to ordering an MVR or PSP report. You can keep the form in your files and/or upload to the MVR Order. PSP reports will NOT be processed until the authorization is emailed to Compliance@MVRcheck.com or attached to the order. We also provide a digital form option for your applicants which saves time.

As an employer, you must notify the applicant in writing – separate from the job application – that an MVR, PSP or Drug Screening report may be used.

MVRcheck provides sample forms for client convenience, including a sample employment disclosure and authorization form, as well as certain state-specific and vendor-required forms where applicable.

Clients are responsible for determining the appropriate forms for their business and for obtaining all disclosures and authorizations required by applicable law before requesting any Services. Clients may use the sample forms provided by MVRcheck or their own legally compliant forms where permitted. Organizations such as vehicle rental companies, insurers, government agencies, and other non-employment organizations should use their own forms where appropriate.
